What is a CDCS Support Planner?

A Consumer Directed Community Supports Planner or CDCS Support Planner for short is a person who has been certified by the Minnesota Department of Human Services (DHS) as understanding the different Community Supports available to a CDCS recipient, person-centered planning, and the process of developing a support plan.

What can a CDCS Support Planner help with?

  • Provide information about CDCS Services and Supports available
  • Help develop a community support plan
  • Help find services and supports
  • Help make arrangements for purchases and delivery of services and supports
  • Help recruit, screen, hire, train, manage, and pay employees that are involved in the community support plan
  • Help monitor the CDCS Support Plan
  • Help troubleshoot issues
  • Provide and help arrange staff training specific to the person's needs.
